All things forms!

As we prepare for the summer, there are a number of required forms that must be completed for each camper. These forms help us care for your camper, ensure safety, and create the best possible camp experience.

All forms are completed through your CampInTouch account or the Campanion app.

How to Access Your Forms

You can access and complete all forms in one of two ways:

1. CampInTouch (Desktop)

  • Log in through our website by clicking “CampInTouch”

  • From your dashboard, navigate to your camper’s forms

  • Upload or complete each required item directly in your account

2. Campanion App (Mobile)

  • Download the Campanion app on your phone

  • Log in using your CampInTouch credentials

  • Access forms, documents, photos, and more on the go

Important Notes

  • All forms must be completed before your camper arrives at camp

  • Some forms may require physician signatures or additional time — we recommend starting early

  • You can save progress and return to forms at any time

  • Forms will not be considered complete until all required sections are submitted

What to Expect

These forms allow our team to:

  • Support your camper’s health and well-being

  • Ensure we are meeting all safety and licensing requirements

  • Better understand your camper before she arrives

  • Camper Profile
    A short set of questions that helps us get to know your camper’s personality, interests, and anything that will help her feel comfortable and confident at camp.

    Camper Photo
    Upload a recent photo of your camper. This helps our staff learn faces quickly and is used for internal rosters and reports.

    Previous Camp Experience (New)
    Lets us know if your camper has attended another camp before and helps us better understand her background.

  • Bunk Requests
    Share bunkmate requests, counselor connections, and any specific needs such as a bottom bunk. We do our best to honor requests while building balanced cabin groups.

    Permission to Share Contact Information (New)
    Opt in to sharing your contact information with other families in your camper’s cabin to help girls stay connected after camp.

  • Activity Preferences
    Campers select activities they are most interested in. Schedules are created based on preferences while also ensuring a well-rounded experience.

    Horseback Riding Form
    Provides details about your camper’s riding experience and interest so we can place her appropriately.

  • Health History
    Standard CampMinder form with medical background, allergies, and important health details.

    Physician’s Examination Form (Requires doctor)
    Must be completed and signed by your camper’s physician to confirm she is cleared to attend camp.

    Immunization Record
    Required documentation of your camper’s immunizations.

    Medication Form
    Details medications your camper will bring, including dosage and instructions.

    Mental & Emotional Health Form
    Helps us better support your camper’s emotional well-being during her time at camp.

    Parent Authorization Form
    Grants permission for participation in camp activities and medical care if needed.

  • HOH Camper/Parent Agreement
    Includes camp policies, liability release, and emergency procedures.

    Arming Your Child Against Abuse
    Important information about camper safety and prevention. (Coming soon)

    Camper Handbook
    Full guide to camp life, expectations, and traditions. (Updated version coming soon)

  • Transportation Information
    Details your camper’s arrival and departure plans.

    Packing List
    A helpful visual guide to ensure your camper is prepared for camp.

    Accommodations & Where to Eat
    Recommendations for families traveling to the area, including places to stay and eat in Kerrville.
